Welcome to Pet Care Veterinary Hospital! On this page we list client forms that you can download at your convenience, print, and fill out before your first visit to the hospital. Please call us at 757.473.0111 if you have any questions about completing the forms, or to make an appointment.
When you come for your first visit, remember to bring your completed forms with you. Also, if you have any prior medical or vaccination records from your previous veterinarian, please bring them with you as well. We look forward to meeting you and having the opportunity to provide the veterinary care your animals deserve.

- New Patient Form—Collects basic information about you and your pet. Fill out this form for all new patients.
- Avian History Form—Gathers important information regarding your bird. Complete this form out for all birds.
- Reptile History Questionnaire—Helps us asses your pet’s health and appropriate husbandry conditions. Complete this form for all reptiles.
- Medical Staffing Form & Consent for Veterinary Drug Use—Lets you know that there are times when the veterinarians and staff are not in the hospital, so you can make an informed decision about leaving your pet for an overnight stay. This also informs you about the medications we may prescribe for your pet. Read and sign this form for all new patients.
